for me i'd say:
Tool - Disgustipated and (-)Ions
that's all i can think about right now
Manic Street Preachers - The Holy Bible.
Truly one of the most disturbing albums ever made. The music is raw and vicious, and the lyrics were almost solely written by Richey Edwards (Who famously disappeared a few months after the album's release after lifelong struggles with depression, self-harm and eating disorders). The songs cover subjescts such as the Holocaust, anorexia and capital punishment, and make for a listening experience that can actually put you in a dark mood. It's an amazing album, and the songs are the best of the band's career but the atmosphere of the record is truly overpowering.

Like a couple have already mentionedL: Pantera's The Great Southern Trendkill, it's just a mental downer. Some Faith No More and Mr. Bungle is obviously pretty disturbing too. I find Alice In Chains self titled album kinda disturbing too, it's got this uncomfortably disturbed vibe to it.
I've still yet to Hear Fantomas though, I'll have to pick up an album.

All albums by Axis of Perdition
The last album - "Deleted scenes from the transition hospital" is a concept album, set in a mental institute & is kinda ambient black metal with a hint of Jazz.
Very messed in the head, but really cool to listen to when wanting to get lost. very much like Devlab, but with more metal

Pink Floyd - The Wall as someone mentioned earlier, pretty intense stuff on that album.
A song that is the ending credits for an anime series called "Monster" by David Sylvian - For the Love of Life. Only a minute and a half long but it's disturbing.
Last track on Lateralus
Rape Song - SYL. Now that I think about it I was absolutely devistated when listening to the whole album for the first time. Never heard something so angry before.
Sane Vs. Normal - Mnemic

Music that I find disturbing..
interesting question. I don't remember, lol.
Perhaps SOAD.
I just don't like the singer's voice at all.
